Terms & Conditions

Phoenix Integrative Counseling LLC

1. Scope of Services​

Phoenix Integrative Counseling LLC provides individual counseling services to adults, with a focus on trauma-informed care and EMDR. Services are offered in-person or via secure telehealth platforms in accordance with Arizona state laws and professional ethics.

2. Eligibility

Services are available to adults age 18 and older. For telehealth sessions, clients must be physically located in Arizona at the time of service, unless otherwise permitted by cross-state licensing agreements.

3. Confidentiality

All counseling sessions are confidential and comply with the Health Insurance Portability and Accountability Act (HIPAA) and Arizona state law. Information will not be disclosed without your written consent, except when required by law (e.g., imminent risk of harm, suspected abuse, court orders, or legal subpoena).

4. Telehealth Services

Virtual sessions are conducted through a secure, HIPAA-compliant platform. Clients using telehealth acknowledge the limitations of online therapy (e.g., technical difficulties, reduced non-verbal cues) and agree to participate from a private, quiet, and safe location.

5. Scheduling and Cancellations

- Counseling sessions are by appointment only.

- Cancellations must be made with at least 24 hours' notice to avoid a cancellation fee.

- Missed appointments or late cancellations may incur the full session fee, which is not billable to insurance.

- Consistent late cancellations or no-shows may lead to discontinuation of services.

6. Fees and Insurance Billing

- Phoenix Integrative Counseling LLC accepts the following insurance plans:

- Aetna

- Blue Cross Blue Shield of Massachusetts

- Carelon Behavioral Health

- Cigna

- Horizon Blue Cross and Blue Shield of New Jersey

- Quest Behavioral Health

- It is your responsibility to confirm your mental health benefits, copays, deductibles, and session limits with your insurer.

- Insurance claims will be submitted on your behalf when applicable. You are responsible for any fees not covered by insurance.

- If insurance denies coverage, or if you are uninsured or choose to pay out of pocket, full payment is due at the time of service.

- Accepted forms of payment include credit/debit card, HSA/FSA cards, and electronic payments.

- A Good Faith Estimate will be provided upon request in accordance with the No Surprises Act.

7. Communication Between Sessions

Communication outside of scheduled sessions is limited to administrative matters (e.g., scheduling or billing). Therapeutic concerns should be addressed during your session. Please allow 24–48 hours for responses during business hours. Phoenix Integrative Counseling LLC does not provide emergency services or crisis care.

8. Emergency Services

If you are experiencing a mental health emergency, please call 911, visit your nearest emergency room, or contact a local crisis line such as:

9. Termination of Services

You may end counseling at any time. Services may also be terminated if:

- Therapeutic progress has plateaued and higher-level care is needed

- There is non-compliance with treatment recommendations

- Boundaries or safety are compromised

- Payments are not received in a timely manner

Referrals will be provided when appropriate.

10. Informed Consent

By beginning services, you acknowledge that you have read, understood, and agreed to these Terms and Conditions. You will also be asked to sign a separate Informed Consent and Privacy Policy before your first session.

11. Changes to These Terms

Phoenix Integrative Counseling LLC may update these Terms and Conditions periodically. Clients will be notified of any significant changes via email or during scheduled sessions.
